Our credentialing process is a very thorough process. Each item listed below is included in each employee caregiver file; completed, verified and signed (where applicable); before the employee is staffed on assignment. We follow all JCAHO and OSHA Standards.
- Resume- Documenting work history
- Employment Application-signed by candidate
- Skills Check List- For each specialty the caregiver will be providing
- Competency Examination results- for each specialty caregiver will be providing
- Two each Professional Reference Release(s)
- Statement of Good Health signed by a Physician (Current within the last year)
- TB/PPD Skin Test (Current within the last year)
- Chest X-ray (current within last 24 months)
- MMR Booster or: a). Rubella Titer, b) Rubeola Titer, c) Mumps Titer
- Varicella Zoster Titer (chicken pox)
- Hepatitis B Vaccination form (with vaccination dates, titer showing immunity, or declination) and / or Hep B Booster
- Copies of Certification (CPR/BLS/ACLS/NALS/etc.)
- Copies of Professional License / Registration
- Copy of Professional License / Registration verification
- Copy of Drivers License
- Copy of Social Security Card
- Completed W-4 and I-9
- Signed Authority to Release Information for: Health Statement, BG Check, and Drug Screen etc.
- Criminal Background Check Results
- Completed OSHA Training Materials
- 10 Panel Drug Screen results
- Performance Appraisal (applicable annually)
